Opinionnaire® Instructions
1. Purpose of Survey
We want to learn what the American public values when it comes to health care. With the 2008 Presidential elections looming on the horizon, we believe it is time to ask the American public what they value and what they want in any new health care system.
We will collect your opinions using this Opinionnaire®, tabulate all the responses, and run periodic reports between now and March 31, 2008. At that time, we will prepare an in-depth report with our findings in order to inform elected officials, presidential candidates and the media. Our goal is to have at least 50,000 people reply during this period.
2. Instructions
To begin, just click on the "Take the survey" link above. If you don't see this link, then you were not automatically logged in and you need to check with the instructions which brought you to this page. At the bottom of the survey will be a SAVE button — click on it to record your opinions. Your responses will not be recorded unless you click the SAVE button.
Your answers are always anonymous.
After you click SAVE, you will receive a note of thanks, and will be presented with links to two websites—National Coalition on Health Care (NCHC.org) and CodeBlueNow! (CodeBlueNow.org)—who are collaborators in this project. Both are national, nonpartisan, nonprofit 501(c)(3), tax exempt organizations.
